Abstract:

According to the WHO Cardiovascular Diseases (CVDs) are the leading cause of death globally, with an estimated 17.9 million people died in 2019 alone representing 32% of all global deaths. (WHO, 2022) It was further stated by the American Heart Association that this reflects a 17.1% increase over the last decade. There have been a total of 523 million recorded cases of CVDs which is a 26.6% increase from 2010. (AHA, 2021) In addition, The Lancet highlighted that majority of the deaths can be found in the Middle- and Low-Income countries. (Dagenais et al., 2020).The digitalization of healthcare domains plays a vital role in today’s healthcare transformation, allowing easy access to relevant data, improving quality of care and delivering value to patients, healthcare practitioners, hospitals, and governments. Thus, Digital Cardiology is described as the use of ICT technologies and platforms to digitalize healthcare processes to support cardiac healthcare practitioners and patients. This is suitable tool that is to be analysed as a solution to help mitigate the issues associated with the cardiology domain.The WHO has also expressed the importance of the use of digital health in the different healthcare domains around the world to elevate the overall quality of healthcare provided to patients. As Per the WHO’s global strategy on digital health it aims to support and respond to the growing needs of countries to implement appropriate digital technologies in accordance with their health priorities and to make progress towards universal health coverage and the health-related Sustainable Development Goals. (WHO, WHO Global strategy on Digital Health 2020-2025, 2021.

According to the World Heart Federation publication of “A Roadmap of Digital Health in Cardiology ” most health challenges faced by healthcare systems around the world can be resolved by the use of digital health technologies. It can further empower patients and healthcare providers, as well as promote universal healthcare coverage, improve patient outcomes, and reduce healthcare costs. (WHF,2022)

Niraj Khatri Sapkota has completed his PhD in Molecular Physiology applications to Cardiovascular Pharmacology at the age of 32 years from Zhejiang University, China. He is now working as an Associate Professor in the Department of Physiology in Chitwan Medical College affiliated to Tribhuvan University, Nepal. Abstract:

Case report: Patient of age 20 years old visited medicine OPD complaining of dyspnoea and dysphagia, diaphoresis with sign of tachypnea (21 breathe per minute) and tachycardia of 150 bpm with systolic and diastolic blood pressure 90/60 respectively, he had history of 12 years pneumonia repeated treatment with recent diagnosis of tuberculosis, treated by medication before 6 months, his weight was 25 kg at the moment of visit, in his previous visit to different clinician, suspected to have cancer due to his reduced weight indication, due to dyspnoea symptoms, medication prescribed which he undergone for 15 days were salmeterol, ipratropium, emsolone, fluticasone during this course of medication his shortness of breath exacerbated along with difficulty in swallowing. Symptomatic status of tachycardia (heartbeat of 150), dyspnoea, dysphagia and more focused symptom was critical reduction in weight 25 kg in 10 years of time, hence he was referred to Cardiology department for further examination expecting that there must be involved of cardiac problem if not we will rule out it. On visit to cardiology department, Echo test was said to perform, not performed till his this age of 20 years.

Investigation: He underwent transthoracic echocardiography that revealed an EF of 36% with severe global Left Ventricular (LV) hypokinesia. His echo impression revealed Global LV hypokinesia. On the basis of echo test result he was prescribed spironolactone, metoprolol and one antibiotic to prevent from infection improve and strengthens the muscle of heart so that his prognosis of symptoms be amended in assumption to remodel the cardiac system and to enhance easier life survivality.

Conclusion: This is clear undiagnosed case of long term pneumonia treated Global left ventricular hypokinesia, under transthoracic investigation demonstrated LV hypokinesia

Abstract:

Background: In recent years, mobile health apps have grown in popularity as a means for people to track and manage their health. While there is increasing worry that these applications may potentially contribute to the emergence of health anxiety, they can also help to encourage healthy behaviours and provide access to health information.

Objective: The objective of this literature review is to look at available mhealth apps and critically evaluate the compromise between reassurance and anxiety.Methodology: A literature review was carried out to analyse the effects of mhealth apps on the creation of health anxiety within the general population. PubMed and SCOPUS were used to search for relevant articles, and abstracts were screened using inclusion criteria of the terms: mhealth apps; e-Health; healthcare apps; cyberchondria; Health anxiety; illness anxiety disorder. A total of 27 studies were included in the review

Results and discussion: The findings suggest a direct relationship between mobile health app use and health anxiety. The impact of mobile health apps on health anxiety may depend on how they are used - individuals receiving a constant stream of health-related information may trigger unnecessary concern about one's health. The need for more regulation and oversight is identified, which can lead to app quality and safety consistency. There are also concerns about data security and privacy and the resulting "digital gap" for individuals without mobiles or internet access.

Conclusion: While health apps can be valuable tools for managing and tracking health, individuals need to use them in a balanced and informed way to avoid increased anxiety
